I used to route documents for review by printing them, writing on them in red pen, scanning them back, and emailing the scan. It took about 25 minutes per document and the resulting scan was always slightly crooked. The first time I saw someone add a sticky note directly to a PDF and send it back in 2 minutes, I felt genuinely embarrassed about my workflow.
PDF annotations — including sticky notes — are a formal part of the PDF specification. According to Adobe's PDF 32000 standard, text annotations (what most people call sticky notes) are defined as popup objects associated with a position on a page. They store the comment text, the author name, a timestamp, and a colour flag. Every PDF viewer — Adobe Reader, Foxit, Apple Preview, Chrome's built-in viewer — renders them as the familiar note icon, and clicking it opens the comment text.
This tool embeds those standard annotations using pdf-lib, an open-source JavaScript library for PDF manipulation. You click anywhere on a page, type your comment, and that annotation is written into the PDF file structure. The original content of the page is completely untouched — the note sits in an annotation layer on top. Open the downloaded file in any PDF reader and the sticky note will be exactly where you placed it.
